We believe in person-centered long-term planning for the future. Our support brokers will assist you in choosing how you live, work and play in your community and create a personalized eligibility-funded Self-Direction budget to support your choices.

Housing Navigation Services

Tired of being on the waitlist for a group home or certified residence? We can assist you in creating community-based housing alternatives utilizing all of the resources available to you. Our Housing Navigator will work with individuals or small groups to create a housing plan and move into their first or next home that meets their unique needs with appropriate long-term services and supports in place.
