Support Broker Services

One size does not fit all!  We believe that everyone should use their social and fiscal resources in a person-centered way to maximize their quality of life and achieve personalized goals.  Our Support Broker services offer person-centered planning for the future, widening your circle of support, getting you active in the community, choosing classes/activities personalized to your interests and creation of personalized eligibility-funded budget to create the life you desire.

A Support Broker* helps a person exercise as much choice and control over their life as possible, using their eligibility-specific funds from Medicaid, NYS and other resources. The Support Broker guides someone through initiation of a self-directed budget, sets up a Personal Resource Account and provides ongoing support.

The Support Broker works with a person who self-directs to:

  • Build their Circle of Support team
  • Educate on available services and community resources
  • Plan a budget for their individualized services
  • Provide training and support on hiring and supervising support staff
  • Develop habilitation plans, when needed
  • Communicate with the Fiscal Intermediary to pay for services

Fee:  $45.08/hour paid out of individuals Personal Resource Account (PRA) funded by their chosen Fiscal Intermediary.
